Please have the following information available to expedite your claim service. 

For an Auto Accident/Loss:

• Date of Accident
• Address of accident (street, city and state)
• Description of Accident
• Driver Involved
• Vehicle Involved – (Year, make, model, & last 4 #’s of VIN #)
• Information of other party/ies involved (include Name, Address, and Phone #)
• Description of the Damage
• Which police department responded?  Was a citation issued?
• Were there any injuries?

For a Property Loss:

• Date of Loss
• Address of Incident (street, city & state)
• How did damage occur?
• Description of Damage
• List of items damaged or stolen
• Police department (if appropriate)

Liability Claim

• Date of Incident
• Address of Incident (Street, City and State)
• How did it happen?
• Who was injured? (Name, Address, and Phone #)
• What was damaged, and who owns it?  (Name, Address, and Phone #)
